Opening keynote - Hospital at home: A health system’s perspective

The COVID-19 pandemic was the mother of innovation for Atrium Health, a Charlotte-based health system with more than 70,000 teammates serving patients at 40 hospitals and hundreds of care locations. Building upon a decade of leading-edge work in the virtual care space, it stood up a hospital-at-home program in a matter of days to help alleviate a crushing demand for bed space. Atrium Health has since evolved the concept into what is now the largest hospital-at-home program in the country, treating a wide variety of medical conditions, such as heart failure, chronic obstructive pulmonary disease, pneumonia, asthma, various infections and post-operative conditions, and partnering with large companies, such as Best Buy Health. It believes virtual care is no longer a “nice to have” – it is a “must-have.” Colleen Hole, who leads Atrium Health’s hospital-at-home program, will share details about how the health system is providing hospital-level care with equal or better outcomes and greater levels of patient satisfaction.

Home-based care: How policy will help shape the future

The ability to deliver care outside the facility setting has expanded rapidly in recent years, leading to significant growth in new care delivery models like home-based primary care. Alongside this momentum, policy trends at the federal and state level have the power to dramatically shape the future of home-based care services – including a renewed focus on advancing accountable care models and unwinding the public health emergency. In this session, Jonathan Gonzalez-Smith will discuss these trends, how they could impact the home-based care landscape, and what it means for HME providers.

The ground is moving: Disruptive partnerships with payers

Laura Williard, vice president of payer relations for AAHomecare, will lead this panel of disruptors in a discussion of how they’re meeting the changing needs of payers in a fast-paced health care environment: Carelon, a subsidiary of Elevance Health, through benefits management solutions that help payers manage risk and deliver “whole-health care”; AdaptHealth through its contract with Humana to provide value-based care to the payer’s Medicare Advantage HMO members; and Modo Health through its clinical and patient engagement tools connecting payers, providers and patients. What are the direct and downstream impacts of their partnerships with payers? How do they expect these partnerships to evolve? And what partnerships do they believe we’ve still yet to see?

In this dynamic session, Jennifer Hutchens and Kim Licata combine their legal and business takes on the hottest trends in health care: managed care, price transparency and digital health, including artificial intelligence. For each of these trends, they’ll provide a level-set, map out the impact and offer ideas for engagement – all with the goal of making you a better partner to larger players in health care. Hutchens, who was recently elected to the board of the American Health Law Association, and Licata, who spent six years leading the corporate law team at Blue Cross Blue Shield of North Carolina, will also leave time for Q&A.

There are two entities behind much of the changing landscape in health care today: so-called “payviders,” and hospitals and health systems. Mitch Steiner and DeAnne Church Hobbs of Premier Inc., a health care improvement company that works with more than 4,400 hospitals and health systems and about 250,000 other providers and organizations, will help untangle the web of vertical integration and joint ventures that are increasingly occurring in health care.
